Discharge Support Workers will be required to work on a rota basis between 7am to 11pm, 7 days per week.
  • 7am to 2:30pm
  • 2.30pm to 11pm
  • 4 on 3 off shift pattern
To be successful in this role, you will need a full driving licence and access to your own vehicle, as well as previous experience in providing person-centred care. This involves dynamically assessing the needs of individuals and supporting their transition from a hospital stay to living as independently as possible in their own homes. You will be required to liaise with other health professionals, and signpost service users and family members to the appropriate additional support, equipment and services to aid their ongoing recovery and long term care needs.
