We create your care plan

We'll create a personalised care plan based on your needs. If you're happy to proceed, we'll agree a start date and introduce your Care Assistant.

You don’t need to have everything figured out before we visit. If you have information such as medication lists or hospital letters, they’re helpful, but not essential. We’ll guide you through everything during the assessment.

Don’t worry. That’s exactly what we’re here for. You don’t need to know what type of care you need before your assessment. We’ll talk through your situation, answer your questions and help you understand the options available. Together, we will create a care plan that’s personalised to your needs, with no pressure to make a decision on the day.

Usually around an hour, although we’ll take as much time as needed.

Yes. Your care assessment is completely free and there’s no obligation to arrange care afterwards.

Many people choose to have a relative, friend or advocate with them during the assessment. We encourage this if it helps you feel more comfortable and supported.

Only if you’re ready. Once you’ve approved your care plan, we’ll agree a suitable start date that works for you.

That’s perfectly normal. Your care plan will be regularly reviewed and can be updated whenever your needs change.
